OBJECTIVE: To determine the supply/demand ratio for procedures related to diagnosis and treatment for chronic kidney disease in the Brazilian National Health System (SUS), in the state of São Paulo, Brazil, 2019. METHODS: This was a descriptive study, using data from the SUS outpatient and hospital information systems. The numbers of medical consultations, diagnostic and chronic kidney disease monitoring tests, performed in the period, were compared with the demand estimation, obtained through ministerial guidelines. RESULTS: Exclusive SUS users were 28,791,244, and individuals with arterial hypertension and/or diabetes mellitus, 5,176,188. The number of procedures performed and the ratio between this number and the needs of the population were 389,414 consultations with nephrologists (85%); 11,540,371 serum creatinine tests (223%); 705,709 proteinuria tests (14%); 438,123 kidney ultrasounds (190%); and 1,045 kidney biopsies (36%). CONCLUSION: In the chronic kidney disease care in the SUS it could be seen simultaneous existence of lack of supply, waste and inadequate screening of important procedures.

Results: The exclusive SUS users were 28,791,244 and the number of individuals with hypertension and/or diabetes, 5,176,188. The number of procedures performed and the relationship between this number and the population's need was 389,414 consultations with a nephrologist (85%), 11,540,371 serum creatinine measurements (223%), 705,709 proteinuria measurements (14%), 438,123 renal ultrasounds (190%) and 1,045 kidney biopsies (36%). Conclusion: In the care of chronic kidney disease in the SUS, there is simultaneously a lack of supply, waste, and deficient tracking of important procedures.


Objetivo: Determinar la relación oferta/necesidad de procedimientos relacionados con el diagnóstico y atención de la enfermedad renal crónica en el SUS del Estado de São Paulo, Brasil, en 2019. Métodos: Estudio descriptivo utilizando datos de los sistemas de información ambulatoria y hospitalaria del SUS. Se comparó el número de consultas médicas y pruebas de diagnóstico y seguimiento de la enfermedad renal realizados con las estimaciones de necesidad recomendadas por directrices ministeriales. Resultados: Los usuarios exclusivos del SUS fueron 28.791.244 y el hipertensos y/o diabéticos, 5.176.188. El número de procedimientos realizados y la relación entre este número y la necesidad de población fue: 389.414 consultas con nefrólogo (85%), 11.540.371 determinaciones de creatinina sérica (223%), 705.709 determinaciones de proteinuria (14%), 438.123 ecografías renales (190%) y 1.045 renales biopsias (36%). Conclusión: En la atención de enfermedad renal en SUS, existe simultáneamente falta de suministro, desperdicio y seguimiento deficiente de los procedimientos importantes.

The present study evaluated the association of food addiction (FA), the change of the BMI/age z-score and the consumption of ultra-processed foods in overweight students undergoing a 16-month, multicomponent intervention in the school environment. FA was investigated using the Yale Food Addiction Scale for Children, and the dietary assessment was estimated using the semi-quantitative FFQ in overweight 9-11-year-old students (BMI/age z-score ≥ 1) of both sexes at their baseline and after the intervention (n 120). Among the schoolchildren, 33·4 % had FA in at least one of the two assessments. The analysis of mixed-effects models to assess the effect of the intervention and the change of the BMI/age z-score between evaluations showed that the occurrence of FA influenced the maintenance of weight (time#FA, ß = 0·30, 95 % CI 0·05, 0·54, P = 0·016). Weight loss was observed only in individuals who did not present FA (BMI/age z-score = -0·3). When evaluating the effect of the intervention and the dietary variables, we verified a reduction in the consumption of sugary milk-based drinks -71·13 kJ (-17 kcal), P = 0·04 only in non-FA students at the end of the study. FA has been identified as an underlying factor with therapeutic relevance, and an enhanced understanding of FA can open new paths for the prevention and management of obesity.

Abstract Introduction: Having national data on chronic dialysis is essential in treatment planning. Objective: To present data of the survey from the Brazilian Society of Nephrology on patients with chronic kidney disease on dialysis in July 2017. Methods: Data was collected from dialysis units in Brazil. The data collection was done using a questionnaire completed online by the dialysis units. Results: Two hundred and ninety-one centers (38.4%) answered the questionnaire. In July 2017, the estimated total number of dialysis patients was 126,583. National estimates of prevalence and incidence rates of dialysis patients per million population (pmp) were 610 (range: 473 in the North region and 710 in the Midwest) and 194, respectively. The incidence rate of new dialysis patients with diagnosis of diabetic nephropathy was 77 pmp. The annual gross mortality rate was 19.9%. Of the prevalent patients, 93.1% were on hemodialysis and 6.9% on peritoneal dialysis, with 31,226 (24%) on the waiting list for renal transplantation. Venous catheter was used as access in 22.6% of patients on hemodialysis. The prevalence rate of positive serology for hepatitis C continued with a tendency to decrease (3.3%). Conclusion: The absolute number of patients and rates of incidence and prevalence on dialysis continued to increase; the mortality rate tended to rise. There were obvious regional and state discrepancies in these rates.


Resumo Introdução: Dados nacionais sobre diálise crônica são fundamentais no planejamento do tratamento. Objetivo: Apresentar dados do inquérito da Sociedade Brasileira de Nefrologia sobre os pacientes com doença renal crônica em tratamento dialítico em julho de 2017. Métodos: Levantamento de dados de unidades de diálise do país. A coleta de dados foi feita utilizando questionário preenchido on-line pelas unidades de diálise. Resultados: 291 (38,4%) centros responderam ao questionário. Em julho de 2017, o número total estimado de pacientes em diálise foi de 126.583. As estimativas nacionais das taxas de prevalência e de incidência de pacientes em tratamento dialítico por milhão da população (pmp) foram 610 (variação: 473 na região Norte e 710 no Centro-Oeste) e 194, respectivamente. A taxa de incidência de novos pacientes em diálise com diagnóstico de nefropatia diabética foi de 77 pmp. A taxa anual de mortalidade bruta foi de 19,9%. Dos pacientes prevalentes, 93,1% estavam em hemodiálise e 6,9% em diálise peritoneal, com 31.226 (24%) em fila de espera para transplante. Cateter venoso era usado como acesso em 22,6% dos pacientes em hemodiálise. A taxa de prevalência de sorologia positiva para hepatite C continua a mostrar tendência para redução (3,3%). Conclusão: O número absoluto de pacientes e as taxas de incidência e prevalência em diálise continuam a aumentar; a taxa de mortalidade tendeu a elevar-se. Há discrepâncias regionais e estaduais evidentes nessas taxas.

The annual incidence rate of patients with diabetic nephropathy was 79 pmp. The annual gross mortality rate was 18.2%. For prevalent patients, 92% were on hemodialysis and 8% on peritoneal dialysis, and 29,268 (24%) were on a waiting list of renal transplant. A venous catheter was the vascular access for 20.5% of the hemodialysis patients. The prevalence rates of positive serology for hepatitis B and C showed a tendency to reduce from 2013 (1.4% and 4.2%, respectively) to 2016 (0.7% and 3.7%, respectively). CONCLUSION: The absolute number and the prevalence and incidence rates of patients on dialysis continue to rise steadily; the gross mortality rate remained stable. Regional inequities are evident in these rates.

The annual incidence rate of patients with diabetic nephropathy was 77 pmp. The annual gross mortality rate was 19%. For prevalent patients, 91% were on hemodialysis and 9% on peritoneal dialysis, 32,499 (29%) were on a waiting list of renal transplant, 37% were overweight/obese, 29% were diabetics, 16% had PTH levels > 600 pg/ml and 26% hemoglobin < 10 g/dl. A venous catheter was the vascular access for 17% of the hemodialysis patients. CONCLUSION: During 2011-2014 the prevalence and incidence rates of patients on dialysis tended to increase, while the gross mortality rate remained stable. In 2014, diabetes was the primary renal disease in 42% of the new dialysis patients.

Conclusion: During 2011-2014 the prevalence and incidence rates of patients on dialysis tended to increase, while the gross mortality rate remained stable. In 2014, diabetes was the primary renal disease in 42% of the new dialysis patients.

INTRODUCTION: National chronic dialysis data have had impact in the treatment planning. OBJECTIVE: To report data of the annual survey of the Brazilian Society of Nephrology about chronic kidney disease patients on dialysis in July 2013 and compare with 2011-12. METHODS: A survey based on data of dialysis units from the whole country. The data collection was performed by using a questionnaire filled out on-line by the dialysis units. RESULTS: Three hundred thirty four (51%) of the dialysis units in the country answered the questionnaire. In July 2013, the total estimated number of patients on dialysis was 100,397. The estimated prevalence and incidence rates of chronic maintenance dialysis were 449 (range: 284 in the North region and 622 in the South) and 170 patients per million population, respectively. The estimated number of new patients starting dialysis in 2013 was 34,161. The annual gross mortality rate was 17.9%. For prevalent patients, 31.4% were aged 65 years or older, 90.8% were on hemodialysis and 9.2% on peritoneal dialysis, 31,351 (31.2%) were on a waiting list of renal transplant, 30% were diabetics, 17% had PTH levels > 600 pg/ml and 23% hemoglobin < 10 g/ dl. A venous catheter was the vascular access for 15.4% of the hemodialysis patients. CONCLUSION: The absolute number of patients on dialysis has increased 3% per year. The prevalence and incidence rates of patients on dialysis leveled off, while the mortality rate tended to decrease compared with 2012. There was a trend towards a better control of the anemia and PTH levels.

Introdução: Dados nacionais sobre diálise crônica têm tido impacto no planejamento do tratamento. Objetivo: Apresentar dados do inquérito da Sociedade Brasileira de Nefrologia sobre os pacientes com doença renal crônica em tratamento dialítico em julho de 2013 e comparar com dados de 2011- 12. Métodos: Levantamento de dados de unidades de diálise do país. A coleta de dados foi feita utilizando questionário preenchido on-line pelas unidades de diálise. Resultados: Trezentos e trinta e quatro (51%) unidades responderam ao inquérito. Em julho de 2013, o número total estimado de pacientes em diálise foi de 100.397. As estimativas nacionais das taxas de prevalência e de incidência de tratamento dialítico foram de 499 (variação: 284 na região Norte e 622 na Sul) e 170 pacientes por milhão da população, respectivamente. O número estimado de pacientes que iniciaram tratamento em 2013 foi 34.161. A taxa anual de mortalidade bruta foi de 17,9%. Dos pacientes prevalentes, 31,4% tinham idade ≥ 65 anos, 90,8% estavam em hemodiálise e 9,2% em diálise peritoneal, 31.351 (31,2%) estavam em fila de espera para transplante, 30% tinham diabetes, 17% tinham PTH > 600 pg/ml e 23% hemoglobina < 10 g/dl. Cateter venoso era usado como acesso em 15,4% dos pacientes em hemodiálise. Conclusão: O número absoluto de pacientes em diálise tem aumentado 3% ao ano nos últimos 3 anos. As taxas de prevalência e incidência de pacientes em diálise ficaram estáveis, e a taxa de mortalidade tendeu a diminuir em relação a 2012. Houve tendência a melhor controle da anemia e dos níveis de PTH. .

The estimated prevalence and incidence rates of chronic kidney disease on maintenance dialysis were 503 and 177 patients per million population, respectively. The estimated number of new patients starting dialysis in 2012 was 34,366. The annual gross mortality rate was 18.8%. For prevalent patients, 31.9% were aged 65 years or older, 91.6% were on hemodialysis and 8.4% on peritoneal dialysis, 30,447 (31.2%) were on a waiting list of renal transplant, 28.5% were diabetics, 36.6% had serum phosphorus > 5.5 mg/dl and 34.4% hemoglobin < 11 g/dl. A venous catheter was the vascular access for 14.5% of the hemodialysis patients. CONCLUSION: The prevalence and incidence rates of chronic kidney disease patients on dialysis increased, while the mortality rate tended to decrease compared with 2011. The indicators of the quality of maintenance dialysis remained stable with a trend towards decrease in levels of anemia. The data highlight the importance of the census to guide chronic dialysis therapy.

INTRODUCTION: Chronic Kidney Disease (CKD) is common, severe and treatable. Its detection involves low cost tests. AIM: To evaluate the effect of a multidisciplinary (nephrologist, social worker, nurse, nutritionist, and psychologist) intervention comparing clinical and laboratory parameters in patients with CKD. METHODS: A prospective study with 2,151 patients attended at the State Center for Kidney Diseases of the Vale do Paraiba, São Paulo, from February 2008 to March 2011. The kidney function was measured using albuminuria and estimated glomerular filtration rate (eGRF) using the MDRD formula The clinical outcomes were the occurrence of cardiovascular disease (CAD), hospitalization episodes, need of renal replacement therapy (RRT) and death. RESULTS: Participants had a mean (range) age of 62 years (14-101), a mean follow-up of 546 days (90-1540) and the majority was in the stage 3 of CKD (59%). The most common primary diagnoses were hypertension (41.2%) and diabetes (32.4%). Mean blood pressure values at the beginning and at the end of treatment were 143 ± 26 mmHg x 87 ± 14 mmHg and 123 ± 16 mmHg x 79 ± 9 mmHg, respectively (p < 0.001); the eGRF decreased from 58.5 ± 31 ml/min. to 56.3 ± 23 ml/min (p < 0.01). Mean value of proteinuria decreased from 1.04 ± 1.44 g/day to 0.61 ± 1.12 g/day, p < 0.001, and the fasting glicemia decreased from 137 ± 73 mg/dl to 116 ± 42 mg/dl. One hundred and twenty-two patients (5.7%) had a CAD episode, the hospitalization rate was 6.6% (n = 143 patients), 7.3% patients died (n = 156), and 1.1% (n = 23) patients needed to start RRT. The risk of cardiovascular events, hospitalization, or death was inversely related to eGRF, and the rates of these events were low compared with the international literature. CONCLUSION: The multidisciplinary care with well defined targets is effective for the preservation of renal function and reduction in morbidity and mortality of CKD patients.

INTRODUÇÃO: A doença renal crônica (DRC) é uma enfermidade grave, comum e tratável, cuja detecção envolve exames de baixo custo. OBJETIVO: Avaliar o efeito de uma intervenção multidisciplinar (nefrologista, assistente social, enfermeira, nutricionista e psicóloga) em parâmetros clínicos e laboratoriais de pacientes com DRC. MÉTODOS: Estudo prospectivo de 2.151 pacientes atendidos no Centro Estadual de Doenças Renais do Vale do Paraíba, SP, de fevereiro de 2008 a março de 2011. A função renal foi avaliada no início e ao final do seguimento por testes de albuminúria e taxa de filtração glomerular estimada (TFGe) pela fórmula do MDRD. Os desfechos clínicos foram: ocorrência de eventos cardiovasculares (ECV), episódios de hospitalização, necessidade de terapia renal substitutiva (TRS) e óbito. RESULTADOS: A idade média foi 62 anos (variação: 14 a 101), com acompanhamento médio de 546 dias (variação: 90 a 1540), havendo predomínio do estagio três da DRC (59%). Os diagnósticos de base mais comuns foram: hipertensão arterial (41,2%) e diabetes (32,4%). A média da pressão arterial antes e ao final do seguimento foi de 143 ± 26 mmHg x 87 ± 14 mmHg e 123 ± 16 mmHg x 79 ± 9 mmHg, respectivamente (p < 0,001); a TFGe reduziu de 58,5 ± 31 ml/min para 56,3 ± 23 ml/min (p < 0,01). A proteinúria caiu de 1,04 ± 1,44 g/dia para 0,61 ± 1,12 g/dia (p < 0,001); e a glicemia de jejum de 137 ± 73 mg/dl para 116 ± 42 mg/dl. Cento e vinte e dois pacientes (5,7%) apresentaram eventos cardiovasculares, a taxa geral de hospitalizações foi de 6,6% (n = 143 pacientes), foram observados 156 (7,3%) óbitos e 23 (1,1%) pacientes evoluíram para TRS. O risco de ECV, hospitalização e óbito aumentou de forma inversa à TFGe, mas são considerados baixos quando comparados à literatura internacional. CONCLUSÕES: A intervenção multidisciplinar com metas bem definidas é efetiva para preservação da função renal e redução da morbidade e mortalidade de pacientes com DRC.

METHODS: A national survey based on data from the country's dialysis centers. Data collection was performed by using a questionnaire filled out online by the dialysis centers. RESULTS: 340 (53.3%) centers answered the questionnaire. National data were estimated for the overall dialysis population. In July 2010, the estimated total number of patients on dialysis was 92,091. The estimated prevalence and incidence rates of end-stage chronic kidney disease patients on maintenance dialysis were 483 and 100/million population, respectively. The estimated number of patients starting a dialysis program in 2010 was 18,972. The annual crude mortality rate was 17.9%. Of those on maintenance dialysis, 30.7% were aged 65 years or older, 90.6% were on hemodialysis and 9.4% on peritoneal dialysis, 35,639 (38.7%) were on a kidney transplant waiting list, 28% were diabetics, 34.5% had serum phosphorus levels > 5.5 mg/dL, and 38.5% had hemoglobin levels < 11 g/dL. Vascular access was through a venous catheter in 13.6% of the hemodialysis patients. CONCLUSIONS: The number of end-stage kidney disease patients on maintenance dialysis is increasing in Brazil. Data concerning the indicators of the quality of maintenance dialysis improved compared to the prior year, and they highlight the importance of the census to guide chronic dialysis therapy.

Em julho de 2010, o número estimado de pacientes em diálise foi de 92.091. As estimativas nacionais das taxas de prevalência e de incidência de insuficiência renal crônica em tratamento dialítico foram de 483 e 100 pacientes por milhão da população, respectivamente. O número estimado de pacientes que iniciaram tratamento em 2010 foi 18.972. A taxa anual de mortalidade bruta foi de 17,9 por cento. Dos pacientes prevalentes, 30,7 por cento tinham idade igual ou superior a 65 anos; 90,6 por cento estavam em hemodiálise e 9,4 por cento em diálise peritoneal; 35.639 (38,7 por cento) estavam em fila de espera para transplante; 28 por cento eram diabéticos; 34,5 por cento tinham fósforo sérico > 5,5 mg/dL e 38,5 por cento, hemoglobina < 11 g/dL. O cateter venoso era usado como acesso vascular em 13,6 por cento dos pacientes em hemodiálise. CONCLUSÕES: A prevalência de pacientes em diálise tem apresentado aumento progressivo. Os dados dos indicadores da qualidade diálise de manutenção melhoraram em relação a 2009 e destacam a importância do censo anual para o planejamento da assistência dialítica.

Of those on maintenance dialysis, 30.7 percent were aged 65 years or older, 90.6 percent were on hemodialysis and 9.4 percent on peritoneal dialysis, 35,639 (38.7 percent) were on a kidney transplant waiting list, 28 percent were diabetics, 34.5 percent had serum phosphorus levels > 5.5 mg/dL, and 38.5 percent had hemoglobin levels < 11 g/dL. Vascular access was through a venous catheter in 13.6 percent of the hemodialysis patients. CONCLUSIONS: The number of end-stage kidney disease patients on maintenance dialysis is increasing in Brazil. Data concerning the indicators of the quality of maintenance dialysis improved compared to the prior year, and they highlight the importance of the census to guide chronic dialysis therapy.

RESUMO

PURPOSE: To evaluate left ventricular mass (LVM) index in hypertensive and normotensive obese individuals. METHODS: Using M mode echocardiography, 544 essential hypertensive and 106 normotensive patients were evaluated, and LVM was indexed for body surface area (LVM/BSA) and for height2 (LVM/h2). The 2 indexes were then compared in both populations, in subgroups stratified according to body mass index (BMI): <27; 27-30; >/= 30kg/m2. RESULTS: The BSA index does not allow identification of significant differences between BMI subgroups. Indexing by height2 provides significantly increased values for high BMI subgroups in normotensive and hypertensive populations. CONCLUSION: Left ventricular hypertrophy (LVH) has been underestimated in the obese with the use of LVM/BSA because this index considers obesity as a physiological variable. Indexing by height2 allows differences between BMI subgroups to become apparent and seems to be more appropriate for detecting LVH in obese populations.
